This form must be signed by you and a parent or guardian. You will not be
allowed to compete in any meets until this contract is signed and submitted.
Signatures must be in pen. Forged signatures will not be accepted. - Meet Attendance Form: Print out and complete offline. Submit to Mr. Koegel
when finished. You must mark which meets you are able to attend, and which
meets you are unable to attend. If you check No, you must give a legitimate
reason for not being able to do so. The final determination as to what qualifies
as a legitimate reason rests with the coaching staff.
