Required Forms: After reading the files in the table above, you are required to
complete the appropriate forms in the table below.

- Meet Attendance Form: Print out and complete offline. Submit to Mr. Koegel
when finished. You must mark which meets you are able to attend, and which
meets you are unable to attend. If you check No, you must give a legitimate
reason for not being able to do so. The final determination as to what qualifies
as a legitimate reason rests with Mr. Koegel and Coach McGriff. - Contract: Print out and complete offline. Submit to Mr. Koegel when finished.
This form must be signed by you and a parent or guardian. You will not be
allowed to compete in any meets until this contract is signed and submitted.
Forged signatures will not be accepted. - Activity Fee Form & Payment: Print out and complete offline. Mail with a check of
$100 to the Board of Education Office when finished. This form must be signed
by a parent or guardian. You will not be allowed to compete in any meets until
this is completed. You are exempt from paying the activity fee if you are eligible
for the free/reduced-cost lunch program. Forged signatures will not be
accepted. This fee covers all extra-curricular activities for the 2009-2010 school
year. If you have already paid your activity fee this year, you do not need to do
so again.
